| HealthAmerica is pleased to offer a bonus program to all licensed and appointed brokers locating in CPA counties effective October 1 through December 15, 2009. The bonus applies to new business groups with effective dates of November 1, 2009 through January 1, 2010. In addition to standard commissions, you have the opportunity to earn the following bonus payments based on the number of subscribers sold. |

Geisinger's new broker bonus program will increase your earnings starting December 1, 2009! |
Bonus available for new groups enrolled during
December 2009 through February 2010.
- A qualifying group is based on 2 - 50 eligible employees in Adams, Berks, Cumberland, Dauphin, Lancaster, Lebanon, Perry & York counties.
- A qualifying group is based on 2 - 99 eligible employees in all other counties in the service area excluding Lehigh & Northampton.
- Employee group size is determined by the information submitted to our rating and underwriting department for premium calculation.
- $5 extra commission per subscriber entitled to selling agent.
- The commission level will be adjusted to the orginal standard contracted rate upon the group's renewal.
